Welcome

Thank you for choosing coaching with Aviva Kohl. This agreement explains what you can expect from the coaching relationship, including confidentiality, payment, and responsibilities.

Purpose of Coaching

Coaching is a collaborative, goal-focused process designed to help you gain clarity, overcome obstacles, and achieve personal or professional goals. Coaching is not psychotherapy, counselling, legal, or medical treatment

Coaching Relationship

• Coaching is a collaborative partnership. • The Client is responsible for their own decisions and actions. • Either party may end the coaching relationship at any time. • Referrals to other professionals may be recommended when appropriate.

Sessions & Fees

Sessions are held via Zoom unless otherwise agreed. Sessions are 60 minutes. Fee: $350 USD per session.

Preferred Payment Method

Zelle is our preferred method of payment.

Payments made by credit card will incur an additional $10 processing fee per session.

Card on File

All clients are required to maintain a valid credit card on file.

Please complete the Credit Card Authorization Form and return it by email to guardtheirhearts@gmail.com.

Your CVV may be sent by email to guardtheirhearts@gmail.com or by text to +1 (845) 376-5964.

If you already have a card on file, we kindly ask that you complete and return an updated Authorization Form to ensure that your information remains current.

Monthly Billing

Invoices are issued throughout the month, and we kindly ask that all outstanding balances be paid before the 1st of the following month.

Any unpaid balance remaining after this date will be automatically charged to the card on file. Charges made to the card on file will include the applicable $10 credit card processing fee per unpaid session.

If you anticipate any difficulty making a payment on time or need to update your payment information, please contact us as soon as possible.

Cancellation Policy

Please provide at least 24 hours' notice to cancel or reschedule. Missed or late-cancelled sessions will be charged.

Confidentiality & Privacy

Everything discussed during coaching is confidential except where disclosure is required by law or where there is a serious risk of harm. Client records are stored securely and used only for coaching and administrative purposes.
